Please follow these steps for completing an RMA:
- Complete and submit the form for each individual product needing service and/or calibration.
- After you complete the form and before submitting, please print the page and verify the accuracy of each entry.
- Please include the printed copy of the form with the product when shipping.
- An Eagle Eye Power Solutions representative will reach out to you with a case number and shipping instructions.

If you are having difficulties viewing or submitting the form:
- Verify that all fields marked “(required)” are filled out.
- Verify that any installed ad blockers in your browser are disabled and try re-submitting the form.
- Alternatively, print the completed form to a PDF file and attach the file to an email to inquiry@eepowersolutions.com with the subject line of “RMA Form Submission”.
